How these differ from a program
A program is where you go and how often. A service is something layered onto it: a medication that makes withdrawal survivable, a clinician treating the depression underneath the drinking, a session that brings the family into the room, a group that keeps going after discharge.
None of them are chosen instead of a program. They are chosen as part of one, usually at the assessment and sometimes weeks later when it becomes clear something is missing.
When they start
Medication assisted treatment and dual diagnosis care generally begin immediately, because withdrawal and untreated mental illness do not wait. Family therapy starts when the family is ready, which is not always at the same time as the patient. Aftercare and the alumni program are planned before discharge rather than after it, on the grounds that the weeks straight after leaving are the ones people most often come unstuck in.
